Discharge Summaries Back to GP

Create your account, select Allied Health and add your HPCSA number, discipline and practice details.

Credentials are reviewed and your allied health portal is activated.

Frequently Asked Questions

Which allied health disciplines are supported?
All HPCSA-registered allied disciplines: physiotherapy, occupational therapy, speech therapy, audiology, dietetics, biokinetics, podiatry, sonography, clinical associate and orthotics/prosthetics.
How do GPs refer to me on ELMED?
GPs use the Referral Hub in their doctor portal, select Allied Health, choose your discipline and send a referral. It arrives in your inbox with the patient's clinical summary.
What does discharge summary closure look like for the GP?
When you send a discharge summary, the referring GP receives a notification. The summary appears in their patient record under the Referrals tab.
Do session note labels change per discipline?
Yes. A physiotherapist sees Treatment Sessions and Treatment Plan. A dietitian sees Consultations and Meal Plan. The labels adapt to your discipline automatically.
