Community Health Portal
Healthcare at ward level. Recorded and tracked.
The ELMED Community Health Worker Portal is built for trained community health workers. Record community visits, capture vitals, log health education sessions and manage referrals to formal health facilities, all connected to the broader ELMED health network.
Built for Community Health Worker
Everything in the Community Health Portal
Record every household or community visit: patient, date, presenting concerns, vitals and health education provided. Walk-in patients can be registered without a formal account.
Record blood pressure, pulse and temperature at the visit. Readings are stored against the patient record and flagged if abnormal.
Log health education sessions: topic, number of attendees and key messages delivered. Track education coverage across your ward.
Refer patients to clinics, hospitals or specialists with urgency levels: routine, urgent or emergency. Track referral status and follow up on outcomes.
Manage your community patient list. See who is overdue for a visit, who has pending referrals and who was seen this week.
Track visit numbers, referral rates and health education coverage across your ward area from your CHW dashboard.
