How to get on ELMED

Register with your CHW certificate number

Create your account, select Community Health Worker and add your certificate number, facility name and ward or community area.

Admin approves your account

Credentials are reviewed and your community health portal is activated.

Log community visits, capture vitals and manage referrals for your ward from your CHW portal.

Frequently Asked Questions

Do community health workers need formal nursing qualifications to join?
No. CHWs who hold a recognised community health worker certificate and are deployed by a formal health programme can register.
Can CHWs use the portal without internet at the point of care?
The portal is web-based and requires connectivity. Offline-first functionality for low-connectivity settings is on the roadmap.
Can CHWs refer patients to ELMED doctors?
Yes. Referrals from the CHW portal can be routed to ELMED doctors and clinics. The patient receives an ELMED account invitation with the referral.
