Create your account, select Palliative Care and add your HPCSA number, care settings and facility details.

Credentials are reviewed and your palliative care portal is activated.

Add patients, document visits, record advance directives and coordinate the care team from your portal.

Frequently Asked Questions

Is the portal suitable for home-based hospice services?
Yes. The portal is web-based and works on any device. Community palliative care nurses use it for home visit documentation.
Can advance directives be accessed quickly in an emergency?
Yes. Advance directives are flagged prominently on the patient record so any ELMED clinician seeing the patient can access them immediately.
Can patients and families see care notes?
Selected care notes can be made visible to the patient in their portal. Full clinical documentation is visible to clinical team members only.
